They can prescribe medication, provide referrals to other services , and determine your disability status.   • How can I get evaluated?
    Along with self-referrals, we accept referrals from private and group practices, home health and case management agencies, insurance carrier referrals, hospitals, assisted living facilities, nursing homes, rehabilitation programs & facilities, and foster care homes. Contact us at 808- 460-4899 to arrange for services. You will be sent clinical paperwork to complete and an appointment will be setup for an evaluation, either via telehealth or home visit.
  • Am I considered homebound?
    You are considered homebound if you need the help of another person or medical equipment such as crutches, a walker, or a wheelchair to leave your home, or your provider believes that your health or illness could get worse if you leave your home.
  • Do you take insurance?
    In addition to Medicare and Medicaid, we accept all major carriers in the state of Hawaii. Please contact us to inquire further.
  • Is there a copay?
    It is based on your health insurance coverage. Contact us and we can discuss your financial responsibility.
